23rd Anniversary of the Olmstead Supreme Court Decision: How to Help Iowans with Disabilities Discharge Successfully to Lower Levels of Care
Please join us on Tuesday, June 21, 2022, at 2:00 p.m. CST for an uplifting 90-minute Iowa PASRR Webinar that takes place one day ahead of the 23rd Anniversary of the Supreme Court's Olmstead Decision, where we'll address Community Placement Supports / Discharge Planning and PASRR Compliant Care Planning in a unique manner. This webinar will feature the “Transition Guide,” developed by Iowa’s Money Follows the Person (MFP) program, and “Ambassadors” who have been successfully discharged to the community from higher levels of care as part of Iowa’s MFP demonstration grant.
A function of Iowa's PASRR process is that for every PASRR Level II Summary of Findings; there are three types of services identified. These include Community Placement Supports that can, when appropriate and implemented, help an individual successfully transition to a lower level of care and thrive once they leave the nursing facility.
