Community Placement Supports / Discharge Planning / PASRR Compliant Care Planning

A function of Iowa's PASRR process is that for every PASRR Level II Summary of Findings; there are three types of services identified. These include Community Placement Supports (CPS) that can, when appropriate and implemented as part of a well-orchestrated discharge plan, help an individual successfully transition to a lower level of care and thrive once they leave the nursing facility.
